Eligibility
-CCHC accepts Medicaid and most private insurance plans.
-This provider participates in the Vaccines For Children (VFC) program. If your child is 1) less than 19 years old and 2) uninsured, Medicaid-eligible, an American Indian or Alaska Native, or underinsured, they are eligible for free vaccines through the VFC program. Click here for more information.

How to Apply
Call 773-238-4100 M,T,W,F 9:00am-5:00pm, Th 10:00am-6:00pm to schedule an appointment. Please have your insurance information (if applicable) available when calling. You must arrive 15-20 minutes before your scheduled appointment to register, and bring the following:
-Photo ID
-Insurance card, or proof of income for sliding scale fees
-Medical history, including a list of providers you have seen and any relevant medical records
-All medications you are taking
-List of any questions you have for your provider
Mental health services are available at the following locations and times:
-Chicago: M 9:00am-5:45pm, Tu 9:00am-4:30pm, W 9:00am-5:30pm, Th 11:00am-5:30pm, F 9:00am-4:30pm
-Calumet City: M 10:00am-3:30pm, W 10:00am-4:00pm, F 10:00am-3:45pm
-South Holland : M 8:00am-3:30pm, Tu 8:30pm-4:00pm
