Austin Family Health Center provides comprehensive health care services for patients of all ages. They offer:
-Primary care, such as physical exams, immunizations, health screenings, and disease management
-Women's Health, including family planning, STI testing, annual exams, and midwifery
-Adult and pediatric behavioral health
-PCC accepts all major PPO health plans, BCBS HMO plans, and the following Medicaid plans: Aetna Better Health, BCBS Community Health, CountyCare, Family Health Network, Harmony, IlliniCare, Lurie Children's, Meridian, and Molina. Financial counselors can help you apply for Medicaid, Medicare, or marketplace health plans.
-Sliding scale fees based on family size and household income are available to uninsured patients.
-No one will be denied service due to inability to pay.
-This provider participates in the Vaccines For Children (VFC) program. If your child is 1) less than 19 years old and 2) uninsured, underinsured, Medicaid-eligible, or an American Indian or Alaska Native, they are eligible for free vaccines through the VFC program. Click here for more information.

How to Apply
Walk-ins are welcome. Patients without an appointments are seen on a first come, first served basis. An appointment is recommended to increase continuity of care and decrease your wait time. Call 773-378-3347 to schedule an appointment. Same day appointments are available: call around 8:15am to schedule a same day appointment. Please bring the following:
-List of medications
-Most recent paperwork/medical records from previous treating physician or hospital
The PCC patient portal allows PCC patients to view medical records, request refills and tests, schedule appointments, and more. The patient portal is available 24/7.
